DOF Config Tool - cannot create user

Hi- I have been trying to create a user for several days now for the DOF config tool. 

The create user page seems non functional

I just created a user myself and i don't have any issue

I'm using Chrome and you?

Yes using chrome - I fill out the new user fields, press submit. Page reloads with blank entries, no email confirmation received etc

OK tried some different /new credentials - all working ok now.
